Why Membership?

The longer answer: Membership ensures each person who considers Open Hands DPC their primary healthcare home receives the quality of care we think they deserve. Conservative estimates of most conventional practices today are between 1200-1600 patients per provider. To establish and grow a practice where you are known and have a true relationship that shapes your patient-doctor partnership, Dr. Kelli has limited her primary care panel to not exceed 800 people. Our tiered membership system, can offer you affordable care while maintaining an appropriate practice size.

Membership Tier Options

$50 enrollment fee is required for all new members to help pay for the programs needed to provide prescriptions, documentation, communications and support necessary for quality and timely care. $100 max payment per family.

All memberships are billed on a monthly basis via the FlexPerx platform for secure credit card, debit card, FSA or ACH bank draft. Automation minimizes non-patient care time and saves you money. (Further details provided in your patient agreement).

  • 2 in-person visits per year

    This provides for a yearly wellness exam and one other in-person visit you require.

Additional Services at Cost

These are services available to all members in addition to the services included with membership. At times a further discussion of results will be needed and this will require at minimum a telemed visit.

  • This is a visit at The Clinic or The Cabin and is scheduled for 30 minutes - all of which is face-to-face time with Dr. Kelli.

    These visits are scheduled based upon availability and medical necessity.

  • - 20 minute call $25

    - text conversation $15

    - refills $10 (unless a visit is needed)

    - Prior Authorization for medication or imaging starts at $20. The cost will be discussed with you if the request is complicated or prolonged.

  • Mileage is calculated from Mount Sterling, KY at $0.65 per mile round trip.

    These visits are scheduled based upon availability and medical necessity.

Consultation Services

If you have been referred to Open Hands DPC for care outside of creating a primary care partnership, we are happy to have you. These services include OMT, Sports Physicals, Work Physicals, DOT Physicals, Stress/Lifestyle Management. Payment for all Non-Membership services is due at the time of service. You can learn more about each below:

  • OMT is a hands-on treatment modality that further separates an Osteopathic Physicians from a regular M.D. Through an OMT session we can address acute and chronic pain, lymphatic drainage and more. These visits usually require 35-45 minutes of care.

    - First visit $100

    - Return Visits $80

  • $100 by appointment only

  • - Individual $35

    - Group of 10 or more $20 each

  • Looking for a second opinion or want to have a more in depth conversation than is possible at your regular PCP? You can be seen for a medical consultation.

    Chart Fee: $50 (as discussed below)

    In-Person Visit: $80

    Telemedicine Visits: $35 (includes all forms of telemedicine care)

    This care level does NOT include PCP level of care and as such you will need to list who or where you receive your primary care when signing up.

All initial memberships and non-member consults require a $50 Registration Fee.

You can upgrade your level of membership at any time or simply choose additional services at cost.

Frequently Asked Questions

  • We do not accept any form of insurance. You will pay Open Hands DPC directly for services received from Dr Kelli. However, if your insurance carrier allows and you prefer to use such coverage for things like labs, we will gladly provide an order for you to get completed at the facility of your choosing.

    We have also worked to secure low cost options for imaging through local private imaging centers. Please read our Insurance Breakdown for a deeper explanation.

    Bottom line - Although we do not accept any form of insurance, we strive to keep costs low and often help our patients find the cheapest way to get the best care.

    Of note, at this time KY Medicaid recipients are not legally allowed to directly contract with their physician. Therefore, if you have Medicaid in the state of KY at this time we are not able to care for you.

  • For Primary Care Services, yes.

    Although we initially offered “Services As Needed” and “Membership” status, we have grown and found that in order to keep a high standard of care, including timely scheduling, the predictability of membership is necessary.

    By creating membership tiers, we can offer quality care in a sustainable manner. You can choose from $0.50 - $1.50 per day for your needs. Then, if those needs change you can add services at cost or change your membership level.

  • You will be sent a link from which you can create an account and choose your membership level. Payment will then be set up as a recurring monthly payment.

    Payment for Additional Services purchased beyond those covered in your membership will be required at the time of service and can be paid by cash (exact change), card via Square, Venmo or check (returned checks will add a $20 fee).

  • DPC stands for Direct Primary Care. This is medical care where you pay directly for your care.

    The common form of care in today’s medical world is third-party-payer insurance. DPC removes the middle man and gives you more control on the decisions made for your health.

  • As a Family Physician, Dr. Kelli welcomes people of all ages from 1 week old and up.

  • D.O. is the abbreviation for Doctor of Osteopathy. Like their allopathic (M.D.) colleagues, D.O.s are fully licensed physicians that have completed 4 years of medical school and at least 3 years of residency. As of July 1, 2020, D.O. and M.D. physician residency programs are even overseen by the same accreditation body.

    Traditionally, osteopathy trains physicians to consider the whole person rather than just addressing symptoms. However, in today’s world any well trained physician is taught this importance. Practically speaking, the in depth musculoskeletal training Dr. Keller received, in addition to a holistic approach to health and wellness, provides her with the expertise needed to offer treatment and prevention plans to her patients that go beyond medication alone. Through manual manipulation (OMT), common aches and pains can often be addressed without the need of long term medication or pharmaceuticals at all!

However, when it is necessary, we will work with you to find a capable specialist that fits your medical and financial needs - whether you have insurance or not.

  • Here at Open Hands DPC, we will see patients regardless of their vaccine status as we continue to strive to keep a healthy line of communication open and respectful toward your lifestyle choices.

    As an evidence based part of preventive care, vaccines are proven to keep each of us and our world as a whole less susceptible to serious illnesses. In today’s world we have access to unprecedented amounts of information. This often leads to questions, concerns and even fears.

    At this time, our DPC is not able to affordably provide vaccinations. However, your local health department is a great source for affordable access to most of the recommended vaccines. Others are often available through local pharmacies. We will gladly work with you to help find the best solution for you and your loved ones’ vaccination needs

  • With more personalized care and the needed time to work together to keep you as healthy as possible, you are less likely to require hospitalization for chronic illnesses. Yet, if an emergency arises, viable options are available. The specific answer to this question is rooted in your particular third-party medical coverage or lack thereof.

    At this time, Open Hands DPC is not offering hospital care. Should the need for hospitalization arise, providing our clinic name and number upon admission or presentation to the emergency room will help facilitate open communication between the emergency room and hospital providers and your physician.

    Generally speaking, the DPC model works well for individuals and families that:

    • continue to use a low premium/high deductible insurance plan - Low premium/high deductible (sometimes called catastrophic coverage) allows the low cost of the DPC to keep your anticipated medical expenses low and without surprises - while saving back for “rainy days”. Then, if life throws a curve ball and hospitalization, surgery or other high cost medical event happens you will have saved money to put towards covering the higher deductible (that will only be needed if such an emergency were to happen).

    • have a form of cost sharing plan - Cost sharing plans most often operate on a per medical event basis including hospitalizations.

    • would otherwise be self-pay regardless of where they seek care. Often discounts are available to those in a self-pay situation when discussed with the hospital directly

    In each scenario, if no emergency befalls, then you will simply have saved that money. We encourage you to explore your options and see how you can use your hard earned money best to care for and protect yourself and loved ones!

  • The DPC model can make your regular medical costs cheaper and more predictable - leading to more financial freedom and independence no matter if you are self-employed, without insurance or on the plan of a third party payer.

    Typical insurance requires an expensive monthly premium followed by copays that are often comparable to our DPC monthly fee. These same copays do not count toward the deductible and typically continue to be required once deductible amounts are met. Additionally, a “max out of pocket” must usually be reached before insurance payment covers 100%. Therefore, you may find that a less expensive traditional plan (low premium/high deductible to be used in case of emergencies) is more desirable for you and your family when you have reliable, regular medical care through our DPC.

    Even if you just need our services every once in a while for acute illnesses, our prices and accessibility give you the ability to be seen - and get a doctor’s note if needed - when you feel too sick to get out, can’t get in to be seen at your regular facility, are visiting Kentucky and your out-of-state insurance will not cover or is not accepted close to where you are. This is why we opted to offer tiers of membership and non-membership consultations - it gives YOU MORE OPTIONS and CONTROL of YOUR HEALTHCARE.

    If you are self-employed or currently without insurance, our low cost cash pay model allows you to avoid hidden fees for services or labs.
